Those are different questions, and most operators only ever ask the first one. The second gets asked by whoever visits — an environmental health officer, a fire officer, a licensing standards officer, a brand auditor — and it gets asked once, without notice, with a score attached. If the first time anybody walks your kitchen with an inspector's question set is the day the inspector walks it, you are finding out what is wrong at the worst possible moment.

Most operators never write a question of their own. The suggested frequency is advice printed on the template — you decide the cadence.

Templates work in three layers. Zynthio's default library is visible to every organisation and editable by nobody but us, so it keeps receiving improvements. Clone one into your organisation and you own that copy. Clone it again into a single site and you have a site variant. When somebody starts an audit the most specific version wins — site, then organisation, then default — resolved automatically rather than chosen from a list.

A score you can defend, line by line

Severity is the weight

Each question carries a severity that doubles as its point value: critical is worth 10 points, major 5, standard 3 and minor 1. Of the 417 default questions, 122 are critical, 213 major, 72 standard and 10 minor. Answers can be yes, no or not applicable, a 0 to 5 score, a temperature, a number or a date — and a number or temperature question with no minimum and no maximum is informational: it captures the reading and never scores, never fails. Date questions can carry an expiry in months, so a certificate dated inside its expiry passes on its own.

Weighted sections and Green, Amber, Red

A section scores its points earned over its points available; the overall figure is the weighted average of those section percentages, using the weights on the template. Unanswered, N/A and informational questions leave the denominator entirely rather than scoring zero. Green needs 85% with no major failures at all and no section under 50%. Pass needs 75% with no more than two major failures. Amber is 60% and up, Red is below it. Two knockouts sit above the arithmetic: any critical failure forces Red and caps the score at 59%, and four or more major failures force Red outright.

Critical failures land the same day

A critical failure does not wait for the audit to be finished and emailed round. The moment it is recorded, every manager allocated to that site and every org admin gets an in-app notification and a push — everyone except the auditor, who is standing there and already knows. Alongside it, a corrective action is raised automatically, titled with the question and described with the comment the auditor was required to write, and due the next day. Major actions are due in seven days, standard in thirty and minor in ninety.

Corrective actions that close honestly

Auto-raised actions are self-healing: fix the problem and correct the answer while you are still walking round, and the open action disappears. Leave it failing and the description keeps up with your comment. Closing takes a resolution note, and for critical and major actions it takes a photo too — those cannot be closed on somebody's word. Verification is genuinely four-eyes: an action must be verified by somebody other than the person who completed it, and the platform refuses otherwise. A verified action is closed and locked, though a manager can reopen one that was closed too early.

Site league tables and repeat offenders

The Audit Performance screen ranks score by site, charts the trend over time, and — the more useful half — surfaces the questions that fail repeatedly across the estate. A question failing at one site is a site problem. The same question failing at nine is a process problem, and that is the one worth a week of head-office attention. Anyone with the module sees their allocated sites; org admins see the whole estate. Audits can be marked unannounced, which is recorded on the audit as a flag.

Frozen once complete, and honest about the rest

Completing an audit freezes the score, the band, the section percentages and the threshold settings that produced them, so changing your banding next quarter never quietly rewrites last quarter's history. Two things we will tell you up front rather than let you discover: nothing in the product schedules an audit — the suggested frequency is advice, and there is no next-audit-due date — and an auto-raised corrective action arrives with no owner, so assigning it is a deliberate step on the Audit Actions page. Audits are run by management-level job titles and org admins; ordinary staff have no Audits module at all.

What audit templates does Zynthio ship with?

How is an audit scored?

Every question carries a severity that doubles as its point value: critical is worth 10, major 5, standard 3 and minor 1. A section score is the points earned over the points available in that section, and the overall score is the weighted average of the section percentages. Questions that were not answered, marked N/A, or that are informational drop out of the denominator entirely rather than scoring zero. Only a 0 to 5 question awards part marks, and on those, answers of 0, 1 and 2 are failing observations even though a 2 still earns 40%.

What are the Green, Amber and Red bands?

Green needs 85% or more, no major failures at all, and no single section below 50%. Pass needs 75% with no more than two major failures. Amber is 60% or more, and Red is anything below 60%. Two knockouts override the arithmetic: any critical failure forces Red and caps the score at 59%, and four or more major failures force Red. Two demotions run after that - a Green with a section under 50% drops to Pass, and a Pass with more than two major failures drops to Amber. Org admins can change the thresholds, and changing them does not rescore old audits, because the rules in force are frozen onto an audit when it completes.

What happens when something fails?

Every failed answer raises a corrective action automatically, titled with the question and described with the auditor's comment. The due date comes from the severity: critical is next day, major is seven days, standard is thirty and minor is ninety. A critical failure also notifies immediately - every manager allocated to that site and every org admin gets an in-app notification and a push, everyone except the auditor. Actions are self-healing, so correcting the answer on the way round makes the open action disappear. Closing a critical or major action requires a photo, and verification is genuinely four-eyes: the platform refuses to let the person who completed an action be the person who verifies it.

Can I compare sites against each other?

Yes. The Audit Performance screen shows score by site, the trend over time, and the questions that fail repeatedly across the estate - which is usually the more useful of the two, because a question that fails everywhere is a process problem rather than a site problem. Anyone with the module sees results for their allocated sites; org admins see the whole estate.

Does Zynthio schedule audits for us?

No, and this is the expectation to set before you roll the module out. The suggested frequency printed on each template is advice and nothing else - there is no next-audit-due date in the product and nothing chases an audit that has not been run. The one thing the module does push at you is a critical failure, which alerts site managers and org admins the moment it is recorded.
